Soccer Tryouts!!

6^th, 7^th, and 8^th graders can try out for Soccer on Wednesday, September 9^th. Girls tryouts are from 4 to 4:30 and Boys tryouts are from 4:40 to 5:10.

All players must have online forms completed and a physical dated after May 1, 2015. Please follow these instructions to complete the forms. If student is a 6^thgrader, they will check 7^th grade on the forms as they will be playing on a 7^thgrade team.

 

Please make sure to put their child's legal first name in the fields, not a nickname or name they go by. Their ID number is their lunch number

Under the Instructions tab, parents and guardians can find links to the medical history form and physical form that should be taken with them to the doctor. These cannot be filled out online and should be returned to you or me. The original can be scanned in and emailed to me as well. THESE MUST BE TURNED IN PRIOR TO ANY PHYSICAL PARTICIPATION.

Under the Electronic Participation Forms, parents will find links that will take them to online forms to ensure that their child is eligible to play. There are four forms to fill out, but it should not take too long. The important thing to note is that ALL FIELDS must be filled out. If something does not apply, not applicable should be written, n/a, or write out blank. Something must be put in each box before the form will allow it to be submitted.
